Red spots on the egg are simply blood clots. They result from the rupture of small blood vessels in the hen's ovaries. The blood has nothing to do with reproduction or the hen's health.

Blood clots are caused by a genetic factor that occurs at the beginning or end of the animal's life cycle. Other causes of bloody drops may include vitamin deficiencies in the diet or liver disease. In either case, such eggs are completely safe and can be consumed without concern.
